Pand\u00e9mia, soci\u00e1lna izol\u00e1cia, strach z choroby, vojna na Ukrajine, ekonomick\u00e1 kr\u00edza, za posledn\u00e9 3 roky ve\u013emi v\u00fdrazn\u00e9 okolnosti, ktor\u00e9 \u00a0hlboko zasiahli dospel\u00fdch. Ako to v\u0161ak pozna\u010dilo detsk\u00fa psychiku vid\u00edme najm\u00e4 na stave pr\u00edjmov na detskej psychiatrii. M\u00e1lo l\u00f4\u017eok nest\u00edha s n\u00e1porom r\u00f4znych psychiatrick\u00fdch diagn\u00f3z det\u00ed v r\u00f4znom veku. T\u00e9ma o\u00a0ktorej sme sa rozpr\u00e1vali s\u00a0na\u0161imi detsk\u00fdmi psychiatri\u010dkami N\u00e1rodn\u00e9ho \u00fastavu detsk\u00fdch chor\u00f4b v\u00a0Bratislave, prednostkou Kliniky detskej psychiatrie doc. MUDr. Janou Trebatickou, PhD., <\/strong>a MUDr. Zuzanou Matzovou, PhD.<\/strong><\/p>\n Najm\u00e4 pr\u00edchodom v\u00edkendu, \u010di sviatkom prib\u00fadaj\u00fa na detskom urgentnom pr\u00edjme pr\u00edpady det\u00ed, ktor\u00e9 zdravotn\u00edci dom\u00e1cky pomenovali\u00a0 – \u201esm\u00fatky\u201c. Smutn\u00e9 deti s\u00a0bo\u013eavou du\u0161ou, rovnako v\u0161ak prich\u00e1dzaj\u00fa aj deti so sklonom ku sebapo\u0161kodzovaniu, \u010di <\/strong>suicid\u00e1lne stavy (<\/strong>pokusy o\u00a0samovra\u017edu), ich po\u010det st\u00e1le rastie, \u010d\u00edm si to vysvet\u013eujete?<\/strong><\/p>\n \u00a0<\/strong>Odpoved\u00e1 doc. MUDr. Jana Trebatick\u00e1, PhD.<\/strong><\/p>\n Pand\u00e9mia, soci\u00e1lna izol\u00e1cia, strach z choroby a jej n\u00e1sledkov, n\u00e1sledne n\u00e1vrat do \u0161k\u00f4l a s t\u00fdm spojen\u00fd tlak na deti a ich v\u00fdsledky. \u010ealej sa k\u00a0tomu pridal strach z vojny. Deti sa s kr\u00edzov\u00fdmi situ\u00e1ciami vyrovn\u00e1vaj\u00fa \u0165a\u017e\u0161ie, ke\u010f\u017ee im ch\u00fdbaj\u00fa \u017eivotn\u00e9 sk\u00fasenosti, v\u010faka ktor\u00fdm ich my, dospel\u00ed, dok\u00e1\u017eeme \u013eah\u0161ie prija\u0165, spracova\u0165 a pre\u017ei\u0165. D\u00f4le\u017eit\u00fa rolu m\u00e1 tie\u017e nadmern\u00fd \u010das tr\u00e1ven\u00fd pas\u00edvne vo virtu\u00e1lnom priestore ako aj nezdrav\u00fd obsah, ktor\u00fd tam deti vyh\u013ead\u00e1vaj\u00fa.<\/p>\n Spom\u00ednate \u201edobu kovidov\u00fa\u201c a\u00a0n\u00faten\u00fa izol\u00e1ciu, ako tento fenom\u00e9n ovplyvn\u00ed deti a\u00a0ich n\u00e1vrat do op\u00e4tovn\u00e9ho norm\u00e1lneho \u017eivota?<\/strong><\/p>\n U\u017e v\u00a0priebehu pand\u00e9mie sme registrovali zmenu \u0161trukt\u00fary diagn\u00f3z u\u00a0na\u0161ich pacientov, \u010do s\u00favis\u00ed ur\u010dite s\u00a0rozli\u010dn\u00fdmi \u0165a\u017ekos\u0165ami, obmedzeniami a\u00a0z\u00e1\u0165a\u017eou ktor\u00e9 priniesla spom\u00ednan\u00e1 pand\u00e9mia COVIDu. Patr\u00ed sem \u00famrtie rodi\u010dov a\u00a0pr\u00edbuzn\u00fdch, strata pravideln\u00e9ho re\u017eimu a naru\u0161enie\u00a0biorytmov, obmedzenie soc. kontaktov, zmena re\u017eimu, \u00fastup povinnost\u00ed a\u00a0n\u00e1vykov, ve\u013ek\u00fd \u010das bez dozoru u\u00a0mlad\u0161\u00edch det\u00ed, n\u00e1sledne n\u00e1vrat do \u0161k\u00f4l- ktor\u00fd tak isto sp\u00f4sobil n\u00e1rast psychick\u00fdch \u0165a\u017ekost\u00ed. V\u00a0zmysle n\u00e1rastu hovor\u00edme a\u017e o dekompenz\u00e1cie \u00fazkost\u00ed, depresi\u00ed,\u00a0 sebapo\u0161kodzuj\u00faceho spr\u00e1vania sa, ale aj agres\u00edvnych prejavov. Deti sa po n\u00e1vrate do \u0161koly museli adaptova\u0165 na mnoh\u00e9 zmeny \u2013 re\u017eimu, z\u00e1\u0165a\u017ee, komunik\u00e1cie, znemen\u00fdch vz\u0165ahov. \u010casto sa medzi sebou porovn\u00e1vali, u\u00a0niektor\u00fdch sa zhor\u0161ili v\u00fdsledky v\u00a0\u0161kole, in\u00ed sa spomalili v\u00a0\u0161portov\u00fdch v\u00fdkonoch, in\u00fdm mohla nar\u00e1s\u0165 hmotnos\u0165. V\u0161etky tieto a\u00a0\u010fal\u0161ie okolnosti, ak\u00fdmi s\u00fa vojna na Ukrajine a\u00a0in\u00e9, m\u00f4\u017eu vies\u0165 k\u00a0nepohode a\u00a0k\u00a0vystup\u0148ovaniu probl\u00e9mov.<\/p>\n O\u00a0ak\u00fdch pr\u00edpadoch konkr\u00e9tne hovor\u00edme?<\/strong><\/p>\n Za posledn\u00fd rok-dva ur\u010dite zaznamen\u00e1vame n\u00e1rast najm\u00e4 ak\u00fatnych pr\u00edpadov ako s\u00fa stavy po suicid\u00e1lnych (samovra\u017eedn\u00fdch) pokusoch, zv\u00fd\u0161en\u00fd je v\u00fdskyt sebapo\u0161kodzuj\u00faceho spr\u00e1vania sa, experimentovania a intoxik\u00e1cie alkoholom\u00a0 a psychoakt\u00edvnymi l\u00e1tkami, \u00fazkostn\u00fdch, depres\u00edvnych por\u00fach, por\u00fach pr\u00edjmu potravy a\u00a0 nadu\u017e\u00edvan\u00edm a\u017e rozvojom z\u00e1vislost\u00ed od internetu a soci\u00e1lnych siet\u00ed. V posledn\u00fdch mesiacoch evidujeme pomerne ve\u013ea adolescentov s tzv. disharmonick\u00fdm v\u00fdvinom ide o chronifik\u00e1ciu du\u0161evn\u00fdch \u0165a\u017ekost\u00ed, ktor\u00e1 sa prejavuje chronick\u00fdmi poruchami regulovania svojich em\u00f3ci\u00ed, zlyh\u00e1van\u00edm v \u0161kolskom v\u00fdkone, rozli\u010dne naru\u0161en\u00fdm spr\u00e1van\u00edm, \u0165a\u017ekos\u0165ami s\u00a0medzi\u013eudsk\u00fdmi vz\u0165ahmi s\u00a0rodi\u010dmi aj s\u00a0rovesn\u00edkmi.<\/p>\n My rodi\u010dia v snahe, \u010do najlep\u0161ie vychov\u00e1va\u0165 deti aj nevedome rob\u00edme mnoh\u00e9 chyby. Ktor\u00e9 \u00a0vzorce v\u00fdchovy sa st\u00e1le opakuj\u00fa a \u00a0nesk\u00f4r m\u00f4\u017eu sp\u00f4sobi\u0165 na krehkej psychike det\u00ed probl\u00e9m? Je to n\u00e1ro\u010dn\u00e9 prostredie, alebo aj snaha ma\u0165 z det\u00ed g\u00e9niov? Ve\u013ea kr\u00fa\u017ekov, st\u00e1le by\u0165 najlep\u0161\u00ed\u2026<\/strong><\/p>\n Je ve\u013emi d\u00f4le\u017eit\u00e9 naladi\u0165 sa na potreby svojho die\u0165a\u0165a a\u00a0re\u0161pektova\u0165 jeho individualitu. A\u00a0najm\u00e4 \u00a0neplni\u0165 si svoje neuskuto\u010dnen\u00e9 sny a\u00a0t\u00fa\u017eby prostredn\u00edctvom die\u0165a\u0165a. D\u00f4le\u017eit\u00e9 je vn\u00edma\u0165 die\u0165a a\u00a0u\u010di\u0165 die\u0165a vn\u00edma\u0165 okolit\u00fd svet aj s\u00a0jeho potrebami, by\u0165 k\u00a0druh\u00fdm pozorn\u00fd, citliv\u00fd a\u00a0spr\u00e1va\u0165 sa \u00a0k\u00a0in\u00fdm \u013eu\u010fom s re\u0161pektom. V\u00a0tom by sme mali \u00eds\u0165 de\u0165om pr\u00edkladom.<\/p>\n V\u00a0poslednej dobe prib\u00fadaj\u00fa pr\u00edpady nov\u00e9ho trendu v\u00fdchovy det\u00ed z\u00a0ktor\u00fdch chc\u00fa ma\u0165 rodi\u010dia g\u00e9niov, m\u00e1me dostupn\u00e9 publik\u00e1cie, ktor\u00e9 rodi\u010dov u\u010dia, \u017ee ka\u017ed\u00e9 die\u0165a m\u00f4\u017ee by\u0165 naozaj dokonal\u00e9. V\u00fdchova spo\u010d\u00edva doslova v spar\u0165ansk\u00fdch podmienkach tvrd\u00e9ho v\u00fdcviku, \u010di u\u017e ment\u00e1lneho, alebo fyzick\u00e9ho. Ak\u00fd je poh\u013ead odborn\u00edka na tento fenom\u00e9n? Tak ako ka\u017ed\u00fd extr\u00e9m, predpoklad\u00e1m, \u017ee ani tento nie je v\u00a0poriadku? <\/strong><\/p>\n Tak ako som spom\u00ednala, neexistuje jednotn\u00fd v\u00fdchovn\u00fd \u0161t\u00fdl unifikovan\u00fd na ka\u017ed\u00e9 die\u0165a. Mus\u00edme sa prisp\u00f4sobi\u0165 osobnosti die\u0165a\u0165a a\u00a0pod\u013ea toho v\u00a0niektorej oblasti ubra\u0165, v\u00a0inej zase prida\u0165. Niektor\u00e9 deti potrebuj\u00fa z\u00a0na\u0161ej strany viac trpezlivosti, in\u00e9 d\u00f4slednej\u0161ie dodr\u017eiavanie hran\u00edc, ale v\u017edy by\u00a0 mala v\u00fdchova prebieha\u0165 s\u00a0re\u0161pektom a\u00a0l\u00e1skavos\u0165ou k\u00a0die\u0165a\u0165u.<\/p>\n \u010co by si mal rodi\u010d v\u0161\u00edma\u0165 na spr\u00e1van\u00ed die\u0165a\u0165a, kedy by bolo dobr\u00e9 spozornie\u0165 a h\u013eada\u0165 pomoc. Stane sa, \u017ee rodi\u010d je a\u017e ten posledn\u00fd, ktor\u00fd zist\u00ed, \u017ee sa nie\u010do deje, ten sign\u00e1l d\u00e1va bu\u010f \u0161kola, alebo okolie.\u00a0<\/strong><\/p>\n Rodi\u010dia by mali by\u0165 vn\u00edmav\u00ed ku svojim de\u0165om v\u00a0ka\u017edom ich vekovom obdob\u00ed, tr\u00e1vi\u0165 s\u00a0die\u0165a\u0165om dostatok \u010dasu. \u00a0Mali by pozna\u0165 z\u00e1ujmy die\u0165a\u0165a a\u00a0aspo\u0148 \u010diasto\u010dne aj jeho kamar\u00e1tov. Ak\u00fako\u013evek zmenu spr\u00e1vania zachyti\u0165, pokia\u013e pretrv\u00e1va, rozpr\u00e1va\u0165 sa s\u00a0die\u0165a\u0165om o\u00a0tejto zmene, poradi\u0165 sa so \u0161kolsk\u00fdm psychol\u00f3gom, zisti\u0165, \u010di zmena spr\u00e1vania je aj v\u00a0in\u00fdch prostrediach, \u00a0v\u00a0\u0161kole, u\u00a0kamar\u00e1tov…n\u00e1sledne vyh\u013eada\u0165 psychologick\u00fa a\u00a0alebo psychiatrick\u00fa pomoc.<\/p>\n D\u00e1 sa poveda\u0165, \u017ee pre s\u00fa\u010dasn\u00e9 deti je \u0165a\u017e\u0161ia doba? \u017de nie s\u00fa pripraven\u00e9 na tie r\u00fdchle zmeny, podnety, n\u00e1strahy?<\/strong><\/p>\n \u0164a\u017eko odpoveda\u0165, ka\u017ed\u00e1 doba je \u0161pecifick\u00e1 nejakou n\u00e1ro\u010dnos\u0165ou. Je d\u00f4le\u017eit\u00e9 vedie\u0165 sa prisp\u00f4sobi\u0165 zmen\u00e1m a\u00a0vy\u0165a\u017ei\u0165 z\u00a0nich to najlep\u0161ie \u010do sa d\u00e1. Ako odborn\u00edci vn\u00edmame tento probl\u00e9m u\u00a0det\u00ed v\u0161eobecne. Maj\u00fa zn\u00ed\u017een\u00fa schopnos\u0165 odolnosti a\u00a0schopnosti reagova\u0165 na zmenen\u00e9 situ\u00e1cie, prenech\u00e1vaj\u00fa zodpovednos\u0165 na druh\u00fdch, maj\u00fa nedostato\u010dn\u00e9 zvl\u00e1dacie mechanizmy.<\/p>\n \u010co deti a ich v\u00fdchova za du\u0161evn\u00fdm zdrav\u00edm? Malo by by\u0165 tak\u00e9ho vzdelanie u\u017e z\u00e1kladom na \u0161kol\u00e1ch?<\/strong><\/p>\n Ur\u010dite by bolo pr\u00ednosom, ak by sa na \u0161kol\u00e1ch venovali du\u0161evn\u00e9mu zdraviu vr\u00e1tane u\u010denia sa re\u0161pektuj\u00faceho spolu\u017eitia v\u00a0spolo\u010dnosti.<\/p>\n Va\u0161e oddelenie u\u017e dlhodobo z\u00e1pas\u00ed s\u00a0probl\u00e9mom nedostatku miest, \u010do by ste potrebovali, aby ste vedeli pokry\u0165 s\u00fa\u010dasn\u00e9 po\u017eiadavky na lie\u010dbu detsk\u00e9ho pacienta. <\/strong><\/p>\n Prv\u00fdm krokom, ktor\u00fd m\u00e1me za sebou je zriadenie a spustenie denn\u00e9ho stacion\u00e1ra. T\u00fdch \u010fal\u0161\u00edch krokov, aby to bolo pod\u013ea na\u0161ich predst\u00e1v je pred nami e\u0161te ve\u013ea. Potrebovali by sme oddelen\u00e9 oddelenia samostatne pre mlad\u0161ie deti, star\u0161ie deti, ak\u00fatne deti, samostatne pre diev\u010dat\u00e1 a chlapcov, pre pacientov s PPP ( poruchy pr\u00edjmu potravy). Ur\u010dite v\u00e4\u010d\u0161ie priestory, aby sme mali miestnosti pre komunitu, vybaven\u00fa mini- telocvi\u010d\u0148u, pracovn\u00fa terapiu (ergoterapia, arteterapia, muzikoterapia), miestnos\u0165 pre skupinov\u00e9 terapie a\u00a0relax\u00e1ciu, bezpe\u010dnostn\u00fa miestnos\u0165. Samozrejme d\u00f4stojn\u00e9 prostredie pre person\u00e1l- kuchynku, oddychov\u00fa miestnos\u0165, nehovoriac o\u00a0sanit\u00e1rnych miestnostiach (toaleta, WC),miestnos\u0165 na individu\u00e1lne pohovory pre rodi\u010dov, pre eduk\u00e1cie. Vysn\u00edvanou je z\u00e1hrada alebo terasa s\u00a0priestorom pre n\u00e1v\u0161tevy, \u0161portov\u00e9 hry, posedenie. Technicky kompletne vybaven\u00e9 oddelenie, vzduchotechnika, slne\u010dn\u00e9 clony at\u010f a\u00a0samozrejme dostato\u010dn\u00fd po\u010det vy\u0161kolen\u00e9ho a vzdelan\u00e9ho person\u00e1lu, okrem lek\u00e1rov a\u00a0sestier najm\u00e4 psychol\u00f3gov, arteterapeutov, ergoterapeutov, mo\u017enosti na superv\u00edzie ako prevencia syndr\u00f3mu vyhorenia a\u00a0na zvy\u0161ovanie kvalifik\u00e1cie.<\/p>\n V\u00a0 N\u00e1rodnom \u00fastave detsk\u00fdch chor\u00f4b za\u010dali deti s\u00a0poruchou pr\u00edjmu potravy lie\u010di\u0165 nov\u00fdm pr\u00edstupom v\u00a0dennom stacion\u00e1ri. <\/strong>Lie\u010dba formou denn\u00e9ho stacion\u00e1ra je pre die\u0165a men\u0161ou emo\u010dnou z\u00e1\u0165a\u017eou bez separ\u00e1cie od rodiny, \u010di rovesn\u00edkov. \u010cas\u0165 stravovania prebieha aj doma, rodina je tak ove\u013ea viac z\u00fa\u010dastnen\u00e1 procesu lie\u010dby. Do procesu lie\u010dby sa zap\u00e1ja cel\u00e1 intenz\u00edvnej\u0161ie cel\u00e1 rodina. Tento pr\u00edstup je nesmierne d\u00f4le\u017eit\u00fd pre obnovenie pocitu kompetentnosti a sebavedomia rodi\u010da. Zatia\u013e zaznamen\u00e1vame pozit\u00edvnu sp\u00e4tn\u00fa v\u00e4zbu od pacientov aj ich rod\u00edn \u010do n\u00e1s ve\u013emi te\u0161\u00ed.<\/p>\n \u00a0<\/strong>Stane sa, \u017ee rodi\u010d nepochop\u00ed d\u00f4le\u017eitos\u0165 lie\u010dby tejto poruchy? Alebo, \u017ee to neberie ako naozaj \u017eivota ohrozuj\u00facu situ\u00e1ciu?<\/strong><\/p>\n Pri poruch\u00e1ch pr\u00edjmu potravy doch\u00e1dza v\u00a0pomerne kr\u00e1tkom \u010dase k\u00a0mno\u017estvu zdravotn\u00fdch \u0165a\u017ekost\u00ed naj\u010dastej\u0161ie v\u00a0s\u00favise s\u00a0prudkou stratou hmotnosti. Rodi\u010d sa takto dost\u00e1va do lek\u00e1rskej starostlivosti lek\u00e1rov rozli\u010dn\u00fdch profesi\u00ed a\u00a0je na n\u00e1s lek\u00e1roch, aby sme im trpezlivo vysvet\u013eovali o\u00a0ak\u00e9 \u0165a\u017ekosti ide a\u00a0povzbudili rodinu, aby sa neb\u00e1la stigmy psychick\u00fdch \u0165a\u017ekost\u00ed a\u00a0ods\u00fadenia druh\u00fdmi.<\/p>\n Asi je \u0165a\u017eko vysvetli\u0165 mame, ktor\u00e1 je st\u00e1le na di\u00e9te a chudne, \u017ee ich rodinn\u00e9 z\u00e1zemie nie je dobr\u00e9 pre die\u0165a s\u00a0poruchou pr\u00edjmu potravy. <\/strong><\/p>\n V\u017edy by sme mali najsk\u00f4r pozna\u0165 s\u00favislosti. Pre\u010do rodi\u010d dr\u017e\u00ed di\u00e9tu, \u010di a\u00a0ako je t\u00e1to t\u00e9ma v\u00a0rodine komunikovan\u00e1.
